Referrals> Follow Ups
In the Referrals screen you can identify Referrals needing either Biomonitoring or Contact follow Ups.
1. You can click the ‘Follow Ups’ link on the left navigation bar. A table with Follow Ups will be displayed. 2. You can click the Select button for a Referral that needs Follow-Up Biomonitoring. A screen with Follow Up information will be displayed. Any reminders for Follow Up Biomonitoring will be displayed. a. You can click the Create Follow Up Biomonitoring Workflow button and a new workflow will be added to the Workflows table. b. You can click the Decline Follow Up Biomonitoring Workflow button to remove the displayed Follow Up (Note: a dialog box will appear asking if you are sure you want to decline this Follow Up. If you click on the OK button, the Follow Up will be declined and will no longer be displayed. If you click on the Cancel button, the Follow Up will remain). c. You can click the Name link to display referral information for that patient. i. You can click the Select Referral button to display referral information for that patient. ii. You can click the Select Contact Entries button to display contact information for that patient. iii. You can click any Select button to display information about the selected Stages for that patient. iv. You can click the Cancel button to return to the Follow Up information screen. d. You can edit or add the following information: For Veterans Affairs Medical Center: · Facility Name (choose one from the dropdown box) · Address 1 · Address 2 · Address 3 · City · State · Zip/Postal · Country · Contact Name · Contact Email · Contact Phone · Contact FAX For Primary Care Physician: · Name · Phone · FAX Number · Address 1 · Address 2 · Address 3 · City · State · Zip/Postal · Country
e. You can Add or Edit the following Health Factors: · Fragments (Note: You can add additional information if Embedded Fragments Present is checked) · Injury Type · Blast/Explosion Type (Note: you can add additional information if Other is checked) · Fragments in Body · Radiography · Vehicle · Surgery · Laboratory · Unable to Screen (Note: you can add information if Acute Illness or Severe Cognitive Impairment is checked). f. You can add a Workflow by choosing one from the dropdown box and clicking the Add Workflow button. g. You can click the Save button and any changes you made will be saved. h. You can click the Back button and you will be returned to the Open screen. i. You can click the Add Contact button and the New Log screen will be displayed. You can add or edit the following information: · Person Contacted First Name (Note: This field is required for saving the information) · Person Contacted Last Name (Note: This field is required for saving the information) · Date Contact was Made (mm/dd/yyyy) (Note: This field is required for saving the information) · Time (hh:mm) · Method of contact (choose one from the dropdown box) · Person Contacted Telephone Number · Person Contacted Email · Person Contacted Address 1 · Person Contacted Address 2 · Person Contacted Address 3 · Person Contacted City · Person Contacted State · Person Contacted Zip Code + 4 · Person Contacted Country · Reason for Contact · Person Contacted Telephone Number (Note: This field is required for saving the information) · Textual Record of the Contact Information(Note: This field is required for saving the information) · Telemedicine Consultation with Provider? · Telemedicine Consultation with Patient? · Inpatient Evaluation Recommended? · Follow Up Contact Needed?
j. You can click the Save button to save the information. k. You can click the Cancel button to remove the information you entered or edited and you will be returned to the Follow Ups details screen.

3. You can click the Select button for a Referral that needs Follow-Up Contact. A screen with Contact information will be displayed.
a. You can edit or add the following information:
· Person Contacted First Name (Note: This field is required for saving the information)
· Person Contacted Last Name (Note: This field is required for saving the information)
· Date Contact was Made (mm/dd/yyyy) (Note: This field is required for saving the information)
· Time (hh:mm)
· Method of contact (choose one from the dropdown box)
· Person Contacted Telephone Number
· Person Contacted Email
· Person Contacted Address 1
· Person Contacted Address 2
· Person Contacted Address 3
· Person Contacted City
· Person Contacted State
· Person Contacted Zip Code + 4
· Person Contacted Country
· Reason for Contact (Note: This field is required for saving the information)
· Textual Record of the Contact Information(Note: This field is required for saving the information)
· Telemedicine Consultation with Provider?
· Telemedicine Consultation with Patient?
· Inpatient Evaluation Recommended?
· Follow Up Contact Needed?
· Follow Up Contact Required By Date (mm/dd/yyyy)
· Remind Me On (mm/dd/yyyy)
· Follow Up Contact Completed Date (mm/dd/yyyy)
b. You can click the Save button to save the information.
c. You can click the Cancel button to remove the information you entered or edited and you will be returned to the Follow Ups screen.
